This form should be completed by BC personnel shipping anything outside of the U.S.
This form should be completed, prior to international travel, by any BC faculty or staff member covered by at least one of the four categories below:
(1) Faculty and Staff traveling to OFAC-sanctioned destinations.
(2) Faculty and Staff intending to travel with BC-owned IT equipment.
(3) Faculty and Staff currently sponsored by a federally-funded research grant or contract.
(4) Faculty and Staff conducting research or working in a STEM field.
This form should be used by BC personnel sponsoring a Foreign National Postdoc.
This form should be completed by all Foreign National Visting Scholars, Postdocs, and Interns

Request to incur costs prior to fully executed agreement.
PI uses to delegate SOME spending authority.
