In the Office of the Dean of Students, our primary goal is to ensure all students feel safe and supported during their time at Boston College. We encourage students to reach out to us if they, or one of their peers, encounter any situation that makes them feel otherwise. We are here to help.
If you are in immediate danger, do not utilize this report form. Immediately contact Boston College Police Department at 617-552-4444 or call 911. The submission of false reports is a violation of the Code of Student Conduct and will be investigated accordingly.

Student disciplinary records remain on file in the Office of Student Conduct for seven years after a student graduates. See the University’s record keeping policy for information on what will be reported. You can view the policy on line at Student Guide.
Please allow 5-10 business days for processing. Submitting this information authorizes the Office of Student Conduct to release all information requested, as stated in University guidelines.
Please submit a new form for each separate request.
Students may submit an appeal of a decision reached in a conduct matter brought under the Student Code of Conduct, whether the decision was the result of an administrative hearing, a hearing board, or any other conduct proceeding administered under the auspices of Student Affairs. If accepted, an appeal will result in an administrative review of the decision; only in rare circumstances will an appeal result in a new hearing or other new adjudication of the case.
Students may authorize the release of personal information and educational records to individuals and/or agencies of their choice by submitting a form of acknowledgment releasing their information.
