Personal Data Form
- The Personal Data Form must be completed by new faculty members and certain student employees only. We encourage all employees to complete a Personal Data Form either online by returning to the eRecruit Application Homepage and selecting the Boston College New Hire Details link, or by filling out the form in paper and bring it to the HRSC in Room 100, 129 Lake Street. Along with name and address information, the Personal Data Form also includes bank payroll deposit information and federal and state tax withholding information.
- The information contained on the Personal Data Form is required in order to enter new employees into the HR/Payroll System and to grant access to the BC Network.
- Instructions for completing the Personal Data Form are available on the Department of Human Resources website.
- All current Boston College employees should use the Agora application or the Personal Data Update Form to update personal data.
