Personal Data Update Form

The Personal Data Update Form may be used by a current employee to request updates to his/her personal data.

Instructions for completing the Personal Data Update Form are available on the Department of Human Resources website.

Current employees may also update certain personal data directly through the UVIEW Agora application.
