Delta Dental Enrollment Form
- Boston College has two Delta Dental plans: DeltaPremier and DeltaCare. Benefits-eligible employees may enroll in either plan within 60 days of their hire date or during the annual open enrollment period in April.
- To participate in either plan, complete the Enrollment Form and send it to the Benefits Office, 129 Lake Street, Room 140.
