A. Sign up online using the online sign-up sheet.

• Choose one of the following 15 minute time slots:
  • 10:00–10:15 a.m.
  • 10:15–10:30 a.m.
  • 10:30- 10:45 a.m.
  • 10:45- 11:00 a.m.
  • 11:00–11:15 a.m.
  • 11:15- 11:30 a.m.
  • 11:30- 11:45 a.m.
  • 11:45- 12:00 p.m.
  • 12:00–12:15 p.m.
  • 12:15–12:30 p.m.
  • 12:30- 12:45 p.m.
  • 12:45- 1:00 p.m.
  • 1:00–1:15 p.m.
  • 1:15- 1:30 p.m.
  • 1:30- 1:45 p.m.
  • 1:45- 2:00 p.m.
  • 2:00–2:15 p.m.
  • 2:15- 2:30 p.m.
  • 2:30–2:45 p.m.
  • 2:45- 3:00 p.m.

NOTE: When a time slot is full that time will disappear from the drop-down menu. Each time slot is full when 70 people have registered. Please choose from one of the next available time slots. PLEASE NOTE: One Registration Time Slot allowed per person.

B. Remember to print, sign, and bring the Consent Form for the flu shot to the Health Fair.
