SUPPLEMENTARY INSTRUCTIONS FOR SUBMITTING A PAPER APPLICATION

We strongly recommend that you fill out and submit the application online, but a paper application is still available. The online application has a number of “drop down” menus that provide answer options in context. When answering the questions below, please choose from the options indicated so that your paper application will match our electronic version. Entering these items exactly as indicated is important to the correct processing of your application.

FILLING OUT THE APPLICANT INFORMATION SECTION

Nurse Anesthesia Applicants: Please enter “MS” as your Intended Degree/Certificate, “Nurse Anesthesia” as both your Intended Program and as your Intended Specialty Area, and “Nurse Anesthetist” as your concentration.

MS-to-PhD Applicants: Please enter “MS-to-PhD” as your Intended Degree/Certificate, and enter “Traditional Master’s” as your Intended Program. Then, choose from the Speciality/Concentration list below and enter these exactly as indicated.

Master’s Entry Applicants: Please enter “MS” as your Intended Degree/Certificate, and choose “Master’s Entry” as your Intended Program. Then, choose from the Speciality/Concentration list below and enter these exactly as indicated. Please note that options with an asterisk are not available for Master’s Entry applicants.

Traditional Master’s Applicants: Please enter “MS” as your Intended Degree/Certificate, and choose “Traditional Master’s” as your Intended Program. Then, choose from the Speciality/Concentration list below and enter these exactly as indicated.

RN-to-MS Applicants: Please enter “MS” as your Intended Degree/Certificate, and choose “RN-to-MS” as your Intended Program. Then, choose from the Speciality/Concentration list below and enter these exactly as indicated.

MS/MBA Applicants: Please enter “MS” as your Intended Degree/Certificate, and choose “MS/MBA” as your Intended Program. Then, choose from the Speciality/Concentration list below and enter these exactly as indicated.

Master’s Completion Applicants: Please enter “MS” as your Intended Degree/Certificate, and choose “Master’s Completion” as your Intended Program. Then, choose from the Speciality/Concentration list below and enter these exactly as indicated.

In order to be processed, all materials must be submitted with an official School of Nursing cover sheet that includes your full name, social security number (if applicable), and date of birth. This information will be used to match your application to all supplementary materials (transcripts, recommendations, etc.) Failure to include the appropriate cover sheet may delay or prevent your application from being reviewed.
