Supplementary Instruction for Submitting a Paper Application
We strongly recommend that you fill out and submit the application online,
but a paper application is still available. The online application has a number
of “drop down” menus that provide answer options in context. When
answering the questions below, please choose from the options indicated so that
your paper application will match our electronic version. Entering these items
exactly as indicated is important to the correct processing of your application.
Filling out the Applicant Information Section
PhD Applicants: Please enter “PhD” as your Intended Degree/Certificate, and leave the remaining options in this section blank.
Completing the Demographic Information Section (optional)
Please check the appropriate entry. If applicable, you may enter further origin information as follows:
- Black: African-American, Cape Verdean (non-Hispanic), West Indian, Hatian, Latino
- Asian: Cambodian, Chinese, Filipino, Indian Subcontinent, Japanese, Korean, Pacific Islander, Vietnamese
- Native American: American Indian or Alaskan Native, Hawaiian Native, Tribal Affiliation
- Hispanic: African, Cuban, Dominican, Mexican American, Puerto Rican, South and Central American

In order to be processed, all materials must be submitted with an official School of Nursing cover sheet that includes your full name, social security number (if applicable), and date of birth. This information will be used to match your application to all supplementary materials (transcripts, recommendations, etc.) Failure to include the appropriate cover sheet may delay or prevent your application from being reviewed.
