These individual forms often contain important details necessary for completing your application, and we recommend that you review each form in advance to ensure that you have the necessary information to complete all materials by the application deadline.
Application Deadline
PhD Program applications are due December 31, for enrollment the following September
Application Checklist and Download Center
To apply for admission to the PhD program please complete the steps indicated in the checklist below. Your application will not be considered complete until you have submitted all of the required documents.
- Application: online application required, printable application (PDF) available
- $40 Application Fee: can be submitted online or mailed with a cover sheet
- Official Transcripts: from all post secondary institutions attended, including Boston College
- Goal Statements: See cover sheet for details
- Letters
of reference: See PhD reference guidelines
- Official GRE Scores: scores must be current within 5 years of application deadline
- Evidence of a graduate-level statistics course
- Evidence of research and creative work and a writing sample
- A current copy of your Curriculum Vitae (CV)
Additional Requirements for International Students
- Official transcript(s) evaluation (by the Commission on Graduate Foreign Nursing Schools) from all post-secondary institutions at which you have studied
- Official TOELF scores
- CGFNS Examination Results
- NCLEX-RN Examination results/RN licensure
Supplemental Forms
Based on the answers you provide in your application, you may need one or more of the following forms to complete your application:
- Suspended or Revoked License Explanation Form (if you indicated a suspended or revoked license on your application)
- Learning Disability Documentation Form (if you indicated a learning disability on your application)
- Supplementary
Information Form (if you wish to submit any supplementary information
in addition to the standard application)

In order to be processed, all materials must be submitted on official forms
or with appropriate official cover sheets that includes your full name, social
security number (if applicable), and date of birth. This information will be
used to match your application to all supplementary materials (transcripts,
recommendations, etc.)
Failure to include the appropriate form/cover may delay
or prevent your application from being reviewed.
