- Transcripts from all undergraduate and graduate programs and completion of a master's degree in nursing from a program accredited by the NLN or CCNE.
- Current curriculum vitae
- Goal Statement
- Three Letters of Reference
- Statistics course
- Evidence of scholarly writing
- Official GRE scores (within five years)
- Interview with faculty
