To begin employment at Boston College, you must complete the following forms and bring them to the office listed below.

I-9 (PDF)
If being paid weekly, complete Section I and bring, with the appropriate original documentation, to Student Services.
M-4 (Massachusetts Withholding Form) (PDF)
Graduate students with assistantships must bring this form to their Dean's office, or send this completed and signed form to the Human Resources Service Center in More Hall.
Off-Campus Federal Work-Study Hire/Rehire Form (PDF)
For Boston College students who have been awarded Federal Work-Study and want to work under our Off-Campus Federal Work-Study program.
Off-Campus Agency Time Sheet (PDF)
For Boston College students who have been awarded Federal Work-Study and are working under our Off-Campus Federal Work-Study program.
W-4 (Federal Withholding Form) (PDF)
Graduate students with assistantships must bring this form to their Dean's office, or send this completed and signed form to the Human Resources Service Center in More Hall.
Direct Deposit Enrollment Form
Direct deposit is the electronic transfer of your current payroll amount from Boston College to the designated account(s) in the bank(s) or credit union(s) of your choice.

CDPP (Controller's Deferred Payment Plan) Application for Graduate and WCAS students only. (PDF)
Dean's List News Release Form (PDF)
Diploma Replacement form if you have lost/damaged a diploma. There is a $40.00 charge for a replacement diploma. (PDF)
Transcript Request can be accessed through your Agora account, or you may complete a Transcript Request (PDF) form and mail or bring it to the Office of Student Services.
Voter Registration Form All students can request that a voter registration form be sent to them. Students may choose between a Massachusetts and a Federal form (for out-of-state students). The form must be returned to your local city or town hall for you to be able to vote.
