PHP Easy Survey Package (phpESP)
The PHP Easy Survey Package is a tool that allows one to easily create simple online surveys. phpESP can be used to create simple surveys with questions that require Yes/No responses, text boxes in which respondents type in replies, scales (for example, rate from 1 to 5), and dropdown lists with a list of possible responses are all easily done with phpESP.
