This form is useful for GP students to keep track of requirements.

Course Approval Form
This form is for courses where the title and description are not obviously focused on inter-national or multicultural issues and are not listed on the pre-approved course list. An approval signature should be obtained from to the Director of Undergraduate Studies for the department the course is under.
Co-curricular and Service Form
A completed co-curricular form is required for the 3 co-curricular and 1 service activities. Print the form, complete each section providing as much detail as possible, and have it signed by the appropriate supervisor or advisor.
Events and Lectures Checklist
You may choose to attend five events and lectures in one semester in order to fulfill one co-curricular activities, other than your service activity . Print out this form, bring it to each event and lecture, complete each section, and have it signed by the appropriate sponsor or representative.
Reflection Project Proposal Form
In the final year students are required to complete a reflection project. This proposal form is due to the Global Proficiency graduate assistant before November 1st of Senior year.
