This form is to be used only for Eagle Bucks/Dining Bucks refunds. If you have lost CASH in one of the vending machines, please call the service number located on the vending machine and follow the directions.
Please review the refund policy for campus vending machines thoroughly, before submitting your request below:
- Refund requests must be made no more than 7 business days after the incident you are reporting. Please do not report incidents that occurred more than 7 business ago.
- Refund request will be processed in the order received usually within 5 days of receipt date. An email message will be sent to requestor once the refund has been processed.
- Refund can only be processed to requestor's Eagle Bucks account. NO CASH REFUNDS WILL BE PROVIDED.
- Machine malfunctions will be verified before refund is authorized. We reserve the right to deny a refund if situation cannot be verified.
- Incomplete refund requests will not be accepted.
