Performance Management Forms
For your convenience, the following forms for use in the Performance Management Process are available on the HR web site:

Instructions for Working with the Performance Management Forms
You may either:

  1. Print the form and hand write or type your responses directly on the hard copy of the form or
  2. Save the web document to your computer so that you can edit it electronically in Microsoft Word.This is the recommended option since it gives you more flexibility in terms of space for responses and making various edits. *Note: Both methods require that you forward a hard copy to the appropriate Vice President.
    To print the form and hand write or type your responses:
    1. Open the form in your Internet browser (Netscape or Internet Explorer) on the HR web site
    2. If you are in the HR frames web site, left-click once on the form document in the right frame (if you are not in the frames site, skip this step)
    3. Click on File from the Menu Bar
    4. Select Print Frame... (select Print if you are not in the frames site)
    5. You will now be able to hand write or type your responses directly on the hard copy of the form

    To save the web document to your hard drive for electronic edits:
    1. Open the form in your Internet browser (Netscape or Internet Explorer) on the HR web site
    2. If you are in the HR frames web site, left-click once on the form document in the right frame (if you are not in the frames site, skip this step)
    3. Click on File from the Menu Bar
    4. Select Save As...
    5. In the Save As dialogue box:
      • Select the appropriate folder on your hard drive or department network drive to Save in
      • Type an appropriate name in the File name box
        For example, to save a Performance Management Appraisal Form template, save as file name: perf_appraisal_2005.html (where perf_appraisal is the name of the form and 2005 is the year)
      • Select Save as type HTML Files
    6. Open Microsoft Word
    7. Click on File from the Microsoft Word Menu Bar
    8. Select Open...
    9. Select the form template (for example, perf_appraisal_2005.html) you just saved
    10. In order to keep the template as a blank template, you should now save the html document with a descriptive name including the name of the employee whose appraisal you are completing
      For example, save as: perf_appraisal_2005_silva.html (where silva is the employee's last name)
    11. You will now be able to type your responses and make edits directly on the employee's form
    12. Remember to save your work frequently!
    13. Once completed, you may print and/or e-mail the completed html form to all applicable managers
