Please fill out this form if you would like a summary of your disciplinary record sent to you or someone else. Your record will include all reportable violations and accompanying sanctions during your time at Boston College. Please allow 5-10 business days for processing. Submitting this form authorizes the Office of the Dean of Students to release all information requested, as stated in University guidelines.
