Web Site Access Request

BC provides several options for BC organizations to publish information via the Web. This form allows you to request access to one of the following:

A Collage-Managed Site
A WebIT-Managed Site
An Independent Web Site (managed with neither Collage nor WebIT
A Student Organization Web Site)

Do NOT use this form to request access to any of the following. Instead, follow the links for additional information.

Note: You will be asked to authenticate upon form submission. The person requesting site access must be the person completing this form so that we can record your agreement to the statements below.
DO NOT SUBMIT THIS FORM ON BEHALF OF SOMEONE ELSE.

*Your Name:		Person requesting ability to edit the site.
*Dept/School/Org:		The user's primary BC organization.

Site Information

*Site Name:		The name of the site to which access is requested, i.e. "Student Services"
*Site URL:		The web site home page URL (for existing sites).
*Site Manager:		The person who is the primary site contact and oversees its daily management.
*Site Type:	Collage-Managed Site WebIT-Managed Site Independent Web Site Student Organization Web Site	Select the type of web site.
*WebIT Permissions:	Edit Pages Add/Remove Pages Promote Pages for Review Make Pages Public
Access Notes:		Any notes about certain access restrictions.
*Advisor:		The name of the faculty or staff advisor for the student organization who will be asked to approve your access.

By submitting this form you agree to:

Abide by all University policies regarding the ethical and courteous use information systems, information confidentiality and copyrights as published in the Boston College Policies and Procedures Manual.
Select and post information on the BC web site in accordance with the Guidelines for BC Web Site Information Providers. These guidelines are intended to encourage the maintenance of an accurate, current, and authoritative University information resource. They also outline categories of information that should NOT normally be included in University web sites. Please review the guidelines before submitting this form.
Respond to inquires about the information in a timely manner.

This form will be sent to the Web Support group who will contact the administrative approver for your web site to obtain authorization to grant you access. Once we acquire approval we will normally be able to grant you access within 24 hours (Monday-Friday).
