Using Forms

This page provides information on a generic form template which is available to Infoeagle Information Providers and Personal Web server users (www2.bc.edu). This template can be customized for your individual needs and no CGI programming experience is required. You will, however, need to be familiar with the HTML coding conventions for FORM tags which can be found in most HTML instructional guides.

Below is a link to the form template and a sample form created from that template, accompanied by two links which will show you the HTML source code for each. To download the template for your own use, view the Form Template with your web browser and use the Save As command to save a copy to your hard drive. You may also wish to print out the source code page for reference.

  • The template source code is color coded for easy editing.
  • Text in RED should be edited to customize that text on the form which the user reads. This includes the form labels, directions for filling out the form, etc.
  • Text in BLUE needs to be edited to customize the way the form works. This includes the values for radio buttons, menus, the email of who receives the output from the form etc
