Every attempt has been made to allow easy completion, printing, and submission of our online forms on different platforms and internet browsers. In order to apply online you will need to have Adobe Acrobat Reader (version 6.0 or greater) installed on the computer you are using. Download a free version of Adobe Acrobat Reader.
It is recommended that you use Internet Explorer to submit these forms if you are using Windows.
If you are submitting your application outside the browser window, Internet Explorer 6.0 or greater is needed to work with Adobe 6.0.
If you are submitting your application inside the browser window, Internet Explorer 5.0 or greater is needed.
If you are submitting your application outside the browser window, Netscape 7.1 or greater is needed.
If you are submitting your application inside the browser window, Netscape 6.2 or greater is needed. Netscape 6.0 does not support the completion of Adobe Forms.
We strongly recommend that you use the latest version of Firefox to submit these forms OUTSIDE of the browser if you are using a Mac.
When using Firefox, the forms will open outside of your browser where they can filled in and submitted. Firefox is a secure, standard web browser that can be downloaded for free.
If you are using Safari, the forms must open OUTSIDE of the browser in order for you to successfully complete and submit them.
If you experience problems printing forms, try setting the selection "Print as Image" in the Printer Dialogue box after selecting the print button on the form.
