The role of the advanced provider is critical in addressing risk factor modification for the prevention of chronic diseases. During this session, we review cardiovascular diease with a focus on the relevant risk factors. Strategies and tools for improved outcomes, including partnering with your patients, are presented. We also discuss the One Million Lives campaign and the impact of the advanced provider in reaching target goals.
