Doctoral dissertation guidelines

Forms are to be filled out by the student, signed by the committee members, and the original submitted by the student to the Graduate Program Assistant in in the main office by the date indicated. Forms that do not require signatures may be emailed to us. The office will make a copy of the other forms for the Graduate Program Director. Students should keep a copy of each completed form for their records.
