REMINDER TO INCOMING STUDENTS!
IMMUNIZATION AND HEALTH FORMS MUST BE RETURNED TO HEATLH SERVICES BY FAX 617-552-1671 OR MAIL TO BOSTON COLLEGE, HEALTH SERVICES, CUSHING HALL RM. 119, 140 COMMONWEALTH AVE, CHESTNUT HILL, MA 02467
Immunization compliance is a State requirement. Failure to do so within 30 days from the start of classes will result in a hold on your registration for classes next semester and your student account will be charged a $65 administrative fee. Please email uhs@bc.edu with any questions.
