Substance Abuse Assessment
ALCOHOL AND DRUG EDUCATION PROGRAM
A Substance Abuse Assesment consists of a one time meeting with a counselor from the Alcohol and Drug Education Program. A student sanctioned to a Substance Abuse Assessment must comply with the recommendations provided by the counselor.
SUBSTANCE ABUSE ASSESSMENT REGISTRATION FORM
IMPORTANT: Please be certain of your referral before proceeding. To register for a SUBSTANCE ABUSE ASSESSMENT, please complete the form below. Within approximately one week, you will be contacted with your scheduled appointment date and time. This is determined based on the information you provide, so please have your schedule available when filling out the form.
PLEASE NOTE: A $100 administrative fee will be charged to your account for all programs that are a result of a disciplinary or court sanction. If you miss a class or appointment, you must reschedule, and you will be charged for the missed class/appointment in addition to your rescheduled class/appointment.
