CORI Form
the neighborhood center
For all programs through which our volunteers/employees will have access to children the Neighborhood Center must receive a completed CORI (Criminal Offender Record Information) Acknowledgement Form.
Please follow the following instructions:
1) Click here to access the Criminal Offender Record Information (CORI) Acknowledgement Form (word doc).
2) Fill out the form
3) Make a copy of a government issued photographic form of identification (driver's license or passport)
4) EMAIL both the CORI Form and copy of photo id to the Neighborhood Center.
If you prefer to send via mail, please mail to the address on the CORI Form.
