This form serves as initial intent to reserve a booth at the conference. Space is limited and you will be assigned a booth when payment is received. Check should be made out to Boston College School of Nursing, Continuing. To be received no later than November 16, 2007 for inclusion in brochure.

	Contribution for exhibit space includes general lighting, one table, one chair, and admission to exhibit area.
